Medicare Part D Outreach Clinics

Student pharmacists from University of the Pacific’s Thomas J. Long School of Pharmacy have assisted 8,686 beneficiaries over the last 13 years with their Medicare Part D drug plan, helping them save an estimated $8.77 million in out-of-pocket drug costs.

Student pharmacists from University of the Pacific’s Thomas J. Long School of Pharmacy plan to serve Medicare beneficiaries with a series of virtual health fairs this year. With the rising costs of prescription drugs, students’ have assisted 8,686 beneficiaries over the last 13 years with their Medicare Part D drug plan, helping them save an estimated $8.77 million in out-of-pocket drug costs.

Despite needing to transition to a telehealth platform due to the COVID-19 pandemic, Pacific student pharmacists will provide the exact same services of Part D review and medication review. Beneficiaries will be able to schedule an appointment at one of the 10 virtual health fairs. They will then be contacted by Pacific's team a couple of weeks before their appointment date to verify their technology access/comfort and to prep them for the Part D/medication review appointment. On the day of the appointment, beneficiaries will be contacted again by an intervention team and a virtual prescription drug plan and medication review will take place. Ideally, the review will occur through a video/teleconferencing platform, but the team will be prepared for those who can only access services via phone as well.

During the Part D review, patients’ current prescription medications, preferred pharmacy and other personalized factors will be reviewed and Pacific's team will identify the plan(s) that would ‘best’ meet their needs. In addition, patients will have the option to be assisted with new plan enrollment at the time of the appointment. For the medication review service, the beneficiaries’ prescription and over-the-counter medications will be evaluated to ensure they are safe to take together and that there are no concerning effects or risks from use. A personalized medication record will also be created.
